Enrollment forms:

These must be completed and turned into the office prior to the first day of school

• Student Release Form 2019-2020
• Ethnicity Report Form 2019-2020

Field trip driver forms:

(each driver must provide all of the below)

• Volunteer Driver Contract
• Auto insurance declarations page (from your insurance company)
• A copy of the driver’s license
• A copy of their motor vehicle record (obtained through the Department of Motor vehicles)
